Participating Retail Pharmacies: For up to a 34-day supply at retail, you will pay:
Use Medco's pharmacy locator to locate a participating retail pharmacy near you. Mail order: For up to a 90-day supply at mail order, you will pay:
Non-Participating Retail Pharmacies: Reimbursement up to 75% of the Maximum Approved Payment (MAP) less applicable co-pay. You will be responsible for the difference in cost between the amount charged and MAP, plus 25% of the MAP amount after deducting your applicable co-payment. Prescription Drug Reimbursement Forms are available from the DaimlerChrysler Service Center (1-800-521-0488). |

Participating Retail Pharmacies: $7.00 co-pay for generic drugs (Kaiser HMO $10). $20 co-pay for brand name preferred (formulary) drugs for each prescription filled at participating pharmacies for all HMO Plans. $35 co-pay for brand name non-preferred (non-formulary) drugs for each prescription filled at participating pharmacies for all HMO Plans (except Kaiser).
Non-Participating Retail Pharmacies and Mail Order: Check with your HMO.
