When do I use mail order?
If you take a prescription medication on a regular basis, you can save over 80% with the PharmaCare Direct. You can get up to a 90-day supply of your covered medication for one mail order co-pay delivered right to your home. Standard shipping is free, and medication is delivered within 10 to 14 days after you place your order.

How do I get started?
The first time you send a new mail service prescription, you will be required to complete a mail service enrollment form and send it to PharmaCare Direct with your original prescription(s) and co-payment(s). A mail service enrollment form will be mailed to your home in June2006. You may also complete the mail service enrollment form on PharmaCare’s Web site, www.pharmacare.com/members or call PharmaCare Customer Service to request that an enrollment form be sent to you. Once you have established your profile you can mail your original prescription(s) and Prescription Order Form to PharmaCare Direct.

How do I change from retail to mail order?
Follow the previous instructions and make sure you order your medications by mail 14 days before they run out.

• Online: You may register online at www.pharmacare.com/members and download an enrollment form to start using PharmaCare Direct for your maintenance prescriptions.
• By fax: Have your physician - fax a prescription to PharmaCare Direct at 1-800-323-0161. Please give your physician your Member ID number found on the front of your PHARMACARE ID card.
  
   

• Online: It's easy: no phone, no forms! All you need is your member ID number (on your PHARMACARE ID card) to order refills on the PharmaCare website. The first time you visit PharmaCare’s website, you will be required to create a User Name and Password. Click on Register Now. Complete the form by entering your information and creating a User Name and Password.
  
  

• By phone: Call to obtain a refill at 1-866-329-4448.
• By mail: Use the refill order form included with the original delivery.

Long-term prescriptions - Mail Order
If you take a prescription medication on a regular basis when you need greater than a 34-day supply, you can get up to a 90-day supply of your covered medication for one mail order co-pay delivered right to your home through PharmaCare Direct, 24 hours a day. Standard shipping is free, and medication is delivered within 10 to 14 days after you place your order.

Short-term prescriptions - Retail
Use a retail pharmacy for short-term prescription needs. You can get up to a 34-day supply of your covered medication for one retail co-pay Use PharmaCare’s pharmacy locator to locate a participating retail pharmacy near you.

Overview
A co-pay is a flat dollar amount paid by the enrollee for each separate prescription order or refill of a covered drug. Reduce your out-of-pocket costs with:

• Mail order: Order drugs through mail order for up to 90-day supply for only one mail order co-pay
• Generic drugs: Opt for generic equivalents of your prescription drugs to get the same health benefits at a lower cost. Ask your doctor or pharmacist whether a generic version of your medication is available and if it would be right for you. A generic equivalent is a medication that contains identical active drug ingredients, dosage, etc. of a brand name prescription drug.

Participating Retail Pharmacies:

Non-Participating Retail Pharmacies:
Reimbursement up to 75% of the Maximum Approved Payment (MAP) less applicable co-pay You will be responsible for the difference in cost between the amount charged and MAP, plus 25% of the MAP amount after deducting your applicable co-payment Direct Member Reimbursement Forms are available from Pharmacare (1-866-329-4448).

If you are eligible for a Health Care Spending Account, login to the PharmaCare members' website to print out a prescription claims history. Submit the history, along with your FSA Health Care Spending Account form, by April 30 to claim for reimbursement.

fter July 1, employees and retirees can set up an account online by visiting www.pharmacare.com/members and clicking on the Register Now link. Enter the information requested on the registration page and create your own User ID and Password to log on. Once you have registered, visit the site and use your User ID and Password to access MyBenefits, PharmaCare’s online member tools to see your own prescription claims history and your family’s co-pays and prescription fill dates. Family members can also register. When refilling prescriptions, you can also refill prescriptions for other family members by entering their prescription number.
